Tunica vaginalis Free Grafts for Closure of Urethrocutaneous Fistulas

In 32 patients a tunica vaginalis free graft was used for closure of urethrocutaneous fistulas (n = 26) or complicated hypospadias repair (n = 6). All patients had at least one operation (average 3.9) for urethral reconstruction prior to this procedure. 12 patients had recurrent fistulas (average 2.2). The technique proved to be very successful. Only 2 patients had to undergo subsequent reoperation for recurrent fistulas. The use of a free tunica vaginalis graft interposed between skin and urethra is our method of choice in the repair of recurrent or complicated urethrocutaneous fistulas.

Although groups at several institutions have long experience with radical perineal prostatectomy (RPP), only few reports of larger series describe associated complications, mostly without reporting management options in detail. We analyzed specific perioperative and postoperative complications of the perineal approach and management strategies thereof.The medical records of 630 patients who underwent RPP between January 1997 and May 2003 were retrospectively reviewed in regard to complications and their management. Transrenal ureteral occlusion using a detachable balloon

Percutaneous transrenal ureteral occlusion using a detachable balloon filled with silicone was performed in 20 patients. The main indications were palliation of large urinary fistulas and as a treatment of last resort for severe dysuria in patients with advanced pelvic malignancies. In 6 patients the contralateral kidney was rendered nonfunctional to obviate the need for bilateral nephrostomy. Due to frequent obstruction of the tube, ureteral blockage in 3 became insufficient. Two dislocated balloons were extracted percutaneously and replaced. In order to achieve effective interruption of urinary flow down the ureter, well-functioning external nephrostomy drainage is necessary.

Transrenal ureteral occlusion: results and problems.

Purpose The effectiveness of transrenal ureteral occlusion was evaluated. Patients and Methods Transrenal ureteral occlusions were performed in 83 ureters of 76 patients. Thirty-one ureters were occluded with use of tissue adhesive, which was secured in place with Gianturco coils in 21. Fifty-two ureters were occluded by means of silicone-filled, detachable latex balloons. Average follow-up was 3.6 months (range, 1 week to 38 months) for patients treated with the tissue adhesive and 7.9 months (range, 1 week to 61 months) for patients treated with the detachable balloons. Urethrocutaneous fistula repair after hypospadias surgery

To evaluate and compare the success rates of simple and layered repairs of urethrocutaneous fistulae after hypospadias repair.The charts of 72 children who developed fistulae after hypospadias repair were reviewed; 39 had a simple closure of the fistula, whereas 32 had a 'pants over vest' repair, in all cases after excluding an impairment of urine outflow.The success rate at the first attempt was 74% for simple closure and 94% for the layered repair; at the second attempt it was 80% and 100%, the difference being statistically significant for both repairs.Although probably far from an optimal technique for repairing urethrocutaneous fistulae, the pants-over-vest repair allows a good success…

Epidermal inclusion cyst of the penis after urethroplasty causing an urethro-cutaneous fistula: a first case report

Penile epidermal cysts are uncommon. We report a pediatric case of epidermal inclusion cyst of the penis after urethroplasty, responsible of the appearance of an urethro-cutaneous fistula. In our opinion, surgical excision of epidermal inclusion cyst after urethroplasty must be performed as quickly as possible to avoid occurrence of postoperative complications.
